Get started with My Residents

Find the residents you care for, check their health status and open their full clinical profile.

Written by Emma Ianson

My Residents gives carers, nurses, managers and Quality and Compliance staff one screen to see how every resident on their ward is doing. When a resident needs attention, you can open their full clinical profile. Once you're familiar with the app, you'll be able to find a resident, read their status and review their health readings with confidence.

You don't need to log in to the app itself. Your Evo platform permissions control what you can access, so if you can't open My Residents, speak to your administrator.

Open My Residents

My Residents opens directly on the resident list. There's no separate login step or landing page inside the app, so you can start reviewing residents as soon as it opens.

Find a resident

The resident list is the home screen. You'll return to it between resident reviews, so it's worth learning how to narrow it down.

Filter by facility and trolley

Use the two dropdowns below the header to show only the residents you need.

  1. Click Home, then select a facility, for example a care home name.

  2. Click Trolley, then select a medication trolley.

The Trolley list only shows the trolleys within the facility you've selected. To see every resident, leave Home set to All facilities. Changing the facility resets the trolley filter to All trolleys, so you won't filter on a trolley that doesn't exist in your new facility.

Search for a resident

Type in the search box to find a resident by name, nickname, room number, facility name or medical condition.

🤓Tip: Typing in the search box clears any active status filter, so your search runs across the whole ward. A nickname works too. For example, typing a nickname finds the resident even when their full name isn't in the search box.

Filter by status

Use the status pills to narrow the list by overall status. Each pill shows a live count.

Pill

What it shows

All

Every resident.

Alert

Residents with at least one metric that needs immediate review.

At Risk

Residents with a metric approaching a threshold.

Normal

Residents with all metrics within range.

Click a pill to apply that filter. Click it again to turn the filter off and return to All.

Read a resident row

Each row gives you a summary without opening the full profile. A row shows:

  • The resident's profile picture, or their initials if there's no picture.

  • The resident's name and preferred name, if one is recorded.

  • The resident's room and facility.

  • The care level, shown as High, Medium or Low.

  • Up to three medical condition tags.

  • A status badge showing Alert, At Risk or Normal, with a matching icon.

  • A Do Not Attempt Resuscitation (DNAR) chip, if applicable.

  • The number of metrics currently flagged.

A coloured left border reinforces the row's status. A red border marks Alert, and an amber border marks At Risk. Rows needing the most attention sit at the top of the list.

Check your search or filter

If no residents match your search or filter, the list shows an empty message suggesting you adjust them.

Open a resident's profile

Click anywhere on a resident's row to open their full profile. The profile replaces the list view. Your search term and status filter are remembered, so they're still applied when you return to the list.

Read the profile banner

The top of the profile is a banner that summarises the resident's current state.

Status bar

If the resident has any metrics that aren't normal, a coloured bar appears at the top of the profile.

  • A red bar shows the number of metrics on alert and asks for immediate review.

  • An amber bar shows the number of metrics at risk and asks you to monitor closely.

Residents with everything normal don't show a status bar.

Identity and key details

Below the status bar, you'll find:

  • The resident's avatar, full name and nickname.

  • Their age and date of birth.

  • Their room and facility.

  • Their National Health Service (NHS) number.

  • A DNAR badge, if applicable.

  • A coronavirus (COVID-19) status badge, shown only when the resident is Positive or Recovered.

Allergies and care plan

At the bottom of the banner, you'll see the resident's known allergies as red pills. If none are recorded, you'll see 'No known allergies'.

Click View Care Plan on the right to open the resident's care plan in Access Point of Care.

Review About and preferences

Click About & Preferences to expand this section. It holds person-centred information about what matters to the resident day to day, rather than clinical data.

Depending on what's recorded for the resident, you may see:

  • About me.

  • What is most important to me.

  • My strengths.

  • Likes and interests, shown as pills.

  • People who are important to me.

  • My relatives, friends and their involvement.

  • Please do and please don't.

  • How and when to support me.

  • How I communicate and how to communicate with me.

  • My wellness.

  • Also worth knowing about me.

Only sections with recorded information appear. If a resident has no information for an item, it doesn't show.

Check clinical information

The clinical information section covers medical conditions, allergies and dietary requirements.

  • Medical conditions appear as badges. If none are recorded, you'll see 'No conditions recorded'.

  • Allergies appear as colour-coded cards, each with one of these severity levels:

    • Severe.

    • Moderate.

    • Mild.

  • Each allergy card shows the allergen, its severity and the reaction to watch for.

  • Dietary requirements appear in a highlighted box when something specific is recorded, for example a pureed diet with thickened fluids. If there are no specific requirements, this row is hidden.

Review vitals and health monitoring

The vitals and health monitoring section is the most detailed part of the profile. It tracks twelve metrics. The section badge shows how many metrics are flagged, or All normal if everything is within range.

Combined risk banner

If two or more metrics are flagged at the same time, a banner appears above the tiles.

  • An amber banner shows the number of metrics flagged at the same time, when fewer than two are at Alert level.

  • A red banner appears when two or more metrics are at Alert level. It urges immediate clinical review.

Metric tiles

Each metric appears as a colour-coded tile showing its current value and a short supporting line. The table below explains what each tile tracks.

Tile

What it tracks

Temperature

Current reading in °C.

Blood Pressure

Systolic and diastolic readings in mmHg.

Pulse

Beats per minute.

O₂ Saturation

Percentage.

Respiration Rate

Breaths per minute.

Fluid Intake

Millilitres consumed, compared with millilitres offered and the daily target.

Nutrition

Meals accepted compared with meals offered, and the amount eaten.

Falls

Incidents in the last 30 days.

Bowel

Days since the last bowel movement, compared with the threshold.

Care Refusals

Refusals in the last 30 days, with any pattern notes.

Medication Trends

As needed (PRN) medication usage, exceptions and pending follow-ups.

Wounds

Active wounds, size trend and latest assessment.

Each tile has a status badge: Alert (red), At Risk (amber) or Normal (teal).

Expand a tile

Click any tile to expand it. An expanded tile:

  • Spans the full width of the grid.

  • Shows a seven-day trend chart for that metric.

  • Shows the resident's personal baseline as a coloured pill below the current value, for example, Baseline: 36.1–37.2 °C.

Only one tile can be expanded at a time. Opening a new tile collapses the previous one. Click the tile again to collapse it.

Examples of expanded tiles

  • Fluid Intake shows today's balance as two progress bars, comparing the amount offered with the amount consumed. It also lists today's individual fluid observations and a seven-day bar chart comparing offered and consumed amounts against the daily target.

  • Bowel shows the resident's seven-day trend of bowel movements. It also shows their usual bowel pattern and how long it has been since their last bowel movement.

Find key contacts

Click Key Contacts to see who to contact about the resident. Up to three cards appear:

  • Primary Contact: name, relationship, phone number and email, if recorded.

  • Power of Attorney (POA): name, relationship, POA type and phone number. This card appears only if a POA is recorded. The POA type is health and welfare, property and finance, or both.

  • General Practitioner (GP): name, practice and phone number.

On a mobile device, tap a phone number to call it.

Understand status colours and terms

Use this glossary to check what the status colours and terms mean.

Term

Meaning

Normal

All readings for this metric are within the expected range.

At Risk

A reading is approaching, or mildly outside, the expected range, so it's worth monitoring.

Alert

A reading is critically outside range and needs immediate attention.

Baseline

The system-defined healthy range for a metric, for example 36.1–37.2 °C for temperature. Baselines are fixed reference values and can't be edited in the app.

DNAR

Do Not Attempt Resuscitation. Shown as a red chip on the resident's row and profile banner when recorded.

PRN

As needed medication, tracked in the Medication Trends tile.

POA

Power of Attorney.

Common questions

Why is a resident at the top of the list?

Residents are sorted by overall status, with Alert first, then At Risk, then Normal. This means the most urgent cases never get buried.

What if I can't find a resident by name?

Try their nickname, room number or a medical condition. The search box also matches these, so one of them may bring up the resident.

Can I change a resident's baseline?

No. Baselines reflect fixed clinical reference ranges, so you can't edit them in My Residents.

Why does a long-standing wound sometimes show red?

Wound status depends on whether the wound is actively managed and whether its assessment is up to date, not only on how long it has existed. A long-standing wound that's well managed shows At Risk. It shows Alert only when its assessment is also overdue.

How do I see the guided tour again?

Click Take a tour in the resident list header. You can also use the three-dot menu option.
