Design custom forms for your care home in Access Point of Care. Describe the form you need, and Researcher drafts it in a format ready to import into your Access Point of Care instance. Every form is a draft that needs review before it goes live.
This guide is for care home administrators and managers who set up forms in Access Point of Care for their home. Designing a form from scratch takes time. This template gives you a structured starting draft, so your time goes on reviewing and refining the form rather than building its structure from the beginning.
By the end, you'll know how to describe the form you need, check the draft, and get it ready for review.
Describe your form
You describe the form in plain English. Researcher asks you to confirm the details before it drafts anything. You'll be asked to confirm:
What the form is for, such as a daily wellbeing check, an incident form or a medication review.
The sections or groups of questions you need.
Which questions must be answered.
Any follow-up questions that should only appear in certain cases, such as a mood follow-up that appears when an earlier answer is given.
Which care plan area the form should link to in Access Point of Care.
How often the form will be completed or reviewed.
Check for a similar form
Before creating a new form, Researcher can check whether a similar form already exists for the same care area. If one does, it asks whether you want a new form or changes to the existing one.
Review your form
The finished form appears in the chat and is saved as a file that you can download. Alternatively, you can ask Researcher to push the form directly to Access Point of Care as a draft form. If you ask for changes, Researcher produces a complete updated version.
⚠️Important: Every form is a draft that needs review before it goes live.
Before you use a form
Check these items before you use a form:
Review every question and any mandatory settings.
Confirm the care plan link is correct.
Publish only once an appropriate.
📌Note: The Access Point of Care connectors are linked to this template by default, so you can start straight away.
