Handover briefing

Find out how you can produce a structured handover for your incoming shift team.

Written by Emma Ianson

Produce a structured handover for the incoming shift team in Access Point of Care. The handover combines Access Point of Care records with the shift notes and reports you upload, so the incoming team has one handover to work from.

Care staff and shift leads who hand over responsibility at the end of a shift can use the handover briefing to pass on what matters most.

Tell Researcher about your shift

Before Researcher builds your handover, tell it the details of your shift:

  • The shift period, for example 'morning shift' or 'last 8 hours'.

  • Your name and role.

  • The location, if relevant.

  • Any residents or incidents you want to prioritise.

Then choose a format for your handover: Markdown, HTML or PDF.

See what the handover includes

Researcher retrieves the Access Point of Care records for your shift and combines them with any files you upload, such as shift notes, incident reports, medication exception logs and appointment records. Overlapping information is merged, so nothing appears twice.

The handover follows this order:

  1. Priority items: anything urgent, unresolved or needing immediate follow-up, or a clear statement that there are no urgent items.

  2. Shift notes.

  3. Incident reports.

  4. Appointment records.

  5. Medication exception logs.

  6. Wellbeing observations.

📌Note: If a category has no information for the shift, the handover says so rather than leaving the heading out.

Researcher doesn't fill gaps. Anything missing from the records is flagged, so you can check it before the handover goes to the incoming team.

Choose the right project

The project you use depends on the handover you need:

  • For a whole-shift or ward-level handover, use one project.

  • For an individual resident handover, use one project per resident.

Link the handover to the Handover Navigator App

Once your handover template is ready, you can link it directly to the Handover Navigator App.

What the incoming team gets

The incoming team gets one handover drawn from several sources, with the most urgent items first. They know what to act on from the start of the shift.

Historical handovers can be viewed for staff who have been on holiday or absent.
