Create forms from template

Steps to create, complete, and save assessment forms from the template library in Access Point of Care.

You can complete forms from the template library to help build the resident’s care plan. Information entered in each form automatically feeds into the care plan. This guide explains how to create and complete a form.

  1. Open the Forms tab
    Select the resident, then click Forms.

  2. Create a new form
    Click Create Form.
    (You may also be prompted to start a form from the profile or actions dashboard.)

  3. Choose a form
    Pick the form you want from the template library.

  4. Complete the assessment
    Follow the guidance in the form and answer the questions.
    Your answers create a score and risk level.

  5. Add strategies and goals
    Use the multi‑select and drop‑down options.
    These will automatically feed into the care plan.

  6. Add any extra hazards (optional)
    Include additional risks if needed.

  7. Set a review date
    Choose a date based on the risk level.
    The system will remind staff to review it on the anniversary.

  8. Save or complete the form

    • Save as unfinished to return later

    • Complete to finalise it and update the care plan

  9. Sign off the form
    Confirm who completed the assessment.

  10. Move to the next assessment
    Start the next suggested form or skip it.
