Edit a form or chart

Steps to edit, update, and publish an existing form or chart in the Access Point of Care template library.

You can update existing forms and charts in Access Point of Care to change their content, layout, or logic. Managers with the right permissions can make these edits through the admin tab.


Access the forms and charts template library

  1. Open the Admin tab.

  2. Click Forms and Charts Templates to view the template library.


Select a form or chart to edit

You can choose the template you want to edit. If it’s published, it is currently in active use.


Edit the form or chart

When editing, you’ll see options to:

  • Check status – see if there are unsaved changes

  • Save Draft – save progress without publishing

  • Publish – use this only when all edits are complete


Template elements

The template name and type cannot be changed for existing templates. Use Designer View to make your edits.


Preview the template

Use Preview View to see how the form will look to staff. You can test it on different devices and orientations.


Modify existing elements

Click any section to update titles, headers, descriptions, or questions. You can also change response types and mark questions as required.


Add a new question

  1. Scroll to the bottom of the template.

  2. Choose a question type (e.g., text box, multiple choice, rating scale).


Set up question details

Enter the question title and description, then choose visibility and layout options.


Add logic and conditions

Use the logic panel to control when a question appears or becomes mandatory based on earlier answers.


Save and publish

Review your edits, save all changes, and click Publish when the form is ready for use.
