Configure forms and charts

Steps to access, search, filter, and understand the forms and charts template library

You can customise forms and charts in Access Point of Care to suit your facility’s needs. Managers with the right permissions can use the admin tab to edit existing templates, create new ones, configure data fields, and assign forms or charts to residents and facilities.


Access the forms and charts template library

  1. Open the Admin tab.

  2. Click Forms and Charts Template.

You’ll see all active templates. Archived forms can also be viewed from this screen.


Search and filter forms and charts

You can quickly locate specific templates using:

  • The slider to browse forms and templates

  • Keyword search to find a particular form

  • Standard template toggle

    • On: shows only standard templates

    • Off: shows both standard and custom templates

  • Type filters to switch between assessments and charts


Understand form and chart details

Each template displays key information:

  • Name

  • Version

  • Status (Published or Draft)

  • Created and last updated dates

  • Type (Assessment or Chart)

  • Available actions (Edit or Configure)

This gives you a clear overview of how each form or chart is set up and what you can adjust.
