Build an assessment template

Steps to create, configure, and publish a new assessment form template

You can create a brand‑new assessment template in Access Point of Care using the template builder. Managers with the right permissions can do this from the admin tab.


Access the template library

  1. Open the Admin tab.

  2. Click Forms and Charts Templates.


Create a new template

  1. Click New Template to start building.

  2. Enter the template name.

  3. Choose the template type (Assessment Form or Chart).


Add a survey title and description

  1. Click Add Question.

  2. Enter a title and description for the survey.

  3. Optionally upload a logo or header.


Set general survey options

You can configure:

  • Staff visibility settings

  • Survey language (default: English)

  • Response limits

  • Survey width


Add a survey logo

Upload a logo and set its size and positioning.


Configure layout options

Choose settings for:

  • Start page

  • Auto‑advance

  • Navigation buttons

  • Progress bar

  • Answer review


Set question settings

You can:

  • Reorder and align questions

  • Add question numbers and change mandatory symbols

  • Set answer length limits

  • Allow text boxes to expand automatically


Configure post‑completion options

Decide what staff see after finishing:

  • A thank‑you message

  • A redirect to an external link


Add pages and questions

  1. Add page titles and descriptions.

  2. Add and configure your questions.


Create conditional questions

  1. Add an initial question (e.g., Yes/No).

  2. Add a follow‑up question.

  3. Use the logic wizard to show or hide questions based on previous answers.


Preview the template

Use Preview to check layout, logic, and how the form appears to staff.


Add expressions

Use expressions to display helpful notes or images without requiring user input.
You can also apply visibility rules for expressions.


Save or publish

  • Save as draft to continue editing later.

  • Publish to make the assessment live.
