You can complete forms from the template library to create or update a resident’s care plan.
Access the Forms Tab
Open the Resident tab.
Select the resident you want to work with.
Click the Forms tab to view all available forms.
Understand Form Review Dates
Each form has a built‑in or manually entered review date.
The Actions tab will prompt staff to review the form on its anniversary.
Create a New Form
Click Create Form to start a new form.
📌Note: You may also be prompted to complete a form from the resident profile or from the Actions dashboard
Select a Form from the Library
Choose the appropriate form from the template list.
🤓Tip: Forms are categorised using a numbering system (e.g., pre‑admission, mandatory risk assessments, care assessments).
Complete the Assessment
Open the assessment template.
Follow the guidance in the form and answer each question.
Scoring and Risk Levels
The system automatically calculates scores based on your answers.
Some factors may trigger high‑risk overrides, changing the risk level.
Document Risk Management Strategies
Add risk‑management strategies using multi‑selects and drop‑downs.
These entries will feed directly into the resident’s care plan.
Set Goals for the Resident
Use multi‑selects to choose relevant goals.
Add free‑text notes to personalise care.
Add Additional Hazards (Optional)
You can categorise any extra hazards related to the resident’s needs.
Set a Review Date
Choose a review date based on risk level (e.g., medium risk may require a one‑month review).
The system will prompt staff on the anniversary of this date.
Save or Complete the Assessment
Save the form if it’s unfinished, or
Complete the assessment to finalise it.
📌Note: Completing the form creates version one and updates the care plan.
Sign Off the Assessment
Confirm and sign off as the person who completed the assessment.
Proceed to the Next Assessment
After submitting, the system will:
Prompt you to start the next assessment in the sequence, or
Allow you to skip it if needed.
