Complete the wound chart on the mobile app

In this article, we explain how to record a wound observation on the mobile app, including how to add body site details, classify the wound, record treatment, upload photographs, and review the full chart in the web application.

You can record a wound observation at any time from a resident’s profile. Each entry flows through to the wound chart in the web application and forms part of the ongoing record. The next review date you set triggers an activity in the app, prompting staff to review the wound regularly.

📌 Note: The wound chart is fully client‑configurable. You can customise fields, add additional options, and tailor how wound information is captured. All wound data is also available within the analytics dashboards in the web application.

Open the wound chart

  1. From the resident’s profile, tap Add Observation.

  2. Choose Wound Chart from the chart library.


Enter wound details

To record the initial wound information:

  1. Choose the body site. The relevant area of the body map opens automatically.

  2. Tap on the exact location of the wound on the body map.

  3. Enter:

    • Where the wound was acquired.

    • The wound type.

    • The date the wound occurred.

    • The time the wound occurred.

  4. Tap Continue.


Complete the wound chart

To add the full wound assessment:

  1. Complete the initial wound classification.

  2. Record whether care was given.

  3. Select the observation type.

  4. Upload a photograph of the wound by choosing one of the following:

    • Select a file stored on your device.

    • Choose Take Photo to capture an image using the device camera.

    📌 Note: If it’s your first time uploading a photo, your device may ask you to allow camera access.

  5. Classify the wound’s depth, width, and repair stage. Guidance appears in the app as you work through the classification.

  6. Enter any factors affecting healing.

  7. Add the treatment information:

    • If treatment is required, enter the dressing type, upload a photo of the dressing, and set a schedule for dressing checks and changes.

    • If no treatment is needed, select no treatment is required.

  8. Enter the next review date.

  9. Add any additional information.

  10. Tap Complete.

Your entry will sync through to the wound chart in the web application.


Review the wound chart in the web application

In the web application, the wound chart displays a continuous record of all wound observations. You can view entries as:

  • A timeline.

  • A table.

  • A body map.

To add a new observation to an existing wound:

  1. Tap New Observation.

  2. Choose one of the following:

    • Create a new wound: To record a different wound at a new location.

    • Go to an existing wound: To complete a review or follow‑up on an existing wound.
