Complete the pain chart on the mobile app

In this article, we explain how to record a pain observation on the mobile app, including how to add body site details, complete a pain assessment, record interventions, and review the chart in the web application.

You can record a pain observation at any time from a resident’s profile. Each observation flows through to the pain chart in the web application, forming a continuous timeline of pain‑related entries. The review date you set drives reminders to the Actions tab, helping staff review the individual's pain regularly.

📌 Note: The pain chart is fully client‑configurable. You can customise how pain is captured, add additional options, and choose which assessment fields are required. Pain data is also available in the analytics tools within the web application.

Open the pain chart

To open the pain chart, follow these steps:

  1. From the resident’s profile, tap Add Observation.

  2. Choose Pain Chart from the chart library.


Enter the pain location

To record where the pain is localised:

  1. Choose the body site where the pain is localised.

    📌 Note: If the pain is non-localised, select No Location to continue adding the pain entry without specifying a body site.

  2. Tap on the position. The body map opens so you can specify the area in more detail.

  3. Enter any additional notes.


Complete the pain assessment

To assess the pain, enter:

  1. Whether the individual can communicate pain successfully.

  2. If so, add a pain rating.

  3. Describe the following:

    • The nature of the pain.

    • The emotional response.

    • The impact.

    • The duration.

    • The interval.

    • Any other contributing factors.


Enter interventions and complete the chart

To record interventions and submit the pain observation:

  1. Add the interventions currently in place to manage the pain.

  2. Enter the next review date.

  3. Tap Complete.

The information flows through to the pain chart in the web application.


Review the pain chart in the web application

In the web application, the pain chart displays a continuous record of all pain-related observations over time. You can review pain observations as:

  • A timeline.

  • A table.

  • A body map view.

To add a new observation to an active pain chart:

  1. Tap New Observation.

  2. Choose the relevant pain chart.

  3. Begin entering details.
