Complete the PURPOSE-T assessment on the mobile app

In this article, we explain how to complete the PURPOSE-T pressure ulcer risk assessment on the mobile app, including initial screening, full assessment, pathway selection, and sign-off.

You can complete the PURPOSE-T pressure ulcer risk assessment from a resident's profile. The system guides you at every stage, uses conditional logic to present only the questions you need to complete, and automatically updates any linked areas of the care plan once you’ve submitted the assessment.

Start the PURPOSE-T assessment

To start the PURPOSE T assessment, follow the steps:

  1. Open the resident’s profile.

  2. Tap Add Observation.

  3. Search for PURPOSE-T and choose it from the library.

  4. Review the on‑screen guidance before beginning the assessment.


Complete the initial screening

To complete the initial screening, follow the steps:

  1. Work through the screening questions, selecting all options that apply.

  2. The system uses conditional logic to decide whether you need to answer any additional screening questions before moving on to the full assessment.

If the initial screening indicates that a full assessment is required, the system automatically takes you to the next stage.


Complete the full assessment

To complete the full assessment, follow the steps:

📌 Note: As you complete each section, the system assigns colour‑coded risk levels. Make sure you review these, as they help determine the correct management pathway.

  1. Specify how often the resident’s position should be changed.

  2. Record the resident’s level of independent movement.

  3. Specify their sensory perception.

  4. Complete the skin assessment by recording any pain, soreness, or discomfort at each listed skin site.

  5. State whether the resident has a previous history of pressure ulcers.

  6. Record whether there’s a perfusion risk.

  7. Record whether there’s a nutritional risk.

  8. Record whether there’s a medical device‑related risk.

  9. Record whether there’s a moisture risk.

  10. Record whether there’s a diabetes‑related risk.

  11. Tap Next to continue.


Select a management pathway

To choose the correct pathway, follow the steps:

  1. Review the overview of the risk levels identified during the assessment.

  2. Select the management pathway checkbox that best matches the resident’s needs using your clinical judgement.

  3. Tap Next to continue.

Once you’ve chosen the pathway, the system maps the result to the resident’s care plan and updates any linked sections automatically.


Sign off the assessment

To sign off and submit the assessment, follow the steps:

  1. Enter your name.

  2. Add your wet signature.

  3. Enter the date the assessment took place.

  4. Enter the time the assessment took place.

  5. Tap Submit Assessment.

The completed assessment is then sent to the PURPOSE-T assessment chart in the web application.
